You may collect the necessary paperwork from the sacristy after each mass .
If you are seeking a baptismal certificate, please apply in writing to Fr Clements at the Parish House, Wilson Rd, Blackley, Manchester M9 8BG and include a self addressed envelope. Fr Clements will require your full name, your date of birth, the names of your parents and an approximate date of baptism. he will also require clear indication as to which church (Mount Carmel or St John Bosco) you were baptised in.
